How Checkboxes Work in Dynamic Forms

Checkboxes in a dynamic form can work in 3 different ways:

  1. Automatic (Mapped) Checkbox - The box checks itself, based on information already in the record. 
  2. Customer-Checked Box - The box is left blank, and the customer checks it themselves while they're signing the document.
  3. Filled by Cemetery - When the Cemetery is generating the form, they manually check them off

Each type of checkboxes is set up by the webCemeteries Project team when your form is built. Once a form is set up, you simply generate the document as you normally would, and the checkbox behaves the way it was configured. A checkbox is always set up as one type or the other; it can't be more than one type at once.

Automatic (Mapped) Checkbox

These checkboxes can be mapped to be checked on or off to specific properties in the record you generate the form from.

For example, if a deceased record has a Military Service Record, the check box for "Veteran" will automatically be checked when you generate the form.

Customer-Checked Box

In this example, when you send a signature request to the customer, the form can be configured to have the customer input additional information, like checking off a box in the form:

   

If your form is setup this way, simply send a signature request like normal, the customer will fill in the details. See our article on How to Electronically sign a Document.

Filled by Cemetery

This is the most Basic configuration. If your form has check boxes configured and they are not the previous 2 options, you will simply check these off yourself when you are in the "Review" stage of generating the form.

Typically, the box will have a light blue "highlight", indicating you can click on the box to check it off:

After checking off the necessary boxes, make sure to select "Save", especially if you have multiple forms being generated at once.
